Overview

Self-management aims to empower people with long-term health conditions to take control of their treatment. It is a set of approaches which help you manage your own health. This could be your physical health, mental health or both. Self-management involves finding out more about your condition, learning new skills to help you manage your health, working in partnership with your health team and ultimately being part of choosing what is right for you. 

During this session we will be joined by three organisations shortlisted for The Alliance Digital Innovator Self Management Award:

  • Cancer Support Scotland
  • Leuchie House
  • Moira Anderson Foundation, Positive Steps Project  
Overall aim

We will be exploring how tech can provide huge benefits to health & social care organisations and the people they support. Digital tools can be a powerful way for patients to self-manage their long-term conditions. This new approach to care has improved patient outcomes by improving compliance with symptom monitoring, improving patient and staff experience as well as use of health resources locally.  We will be talking candidly about the pros and cons of digital self-management and what the sector needs to do next in this space.
